According to a preliminary NASA investigation, that's apparently what happened to the good and faithful MARS GLOBAL SURVEYOR SPACECRAFT, which abruptly fell silent last year after a decade of meticulously mapping the Red Planet.
Somehow, "Oops" just doesn't seem to cover it any longer.
